Off Campus Lunch Privilege Permission forms:

This is for Juniors and Seniors only. If your student would like to leave campus during their lunch, an Off-Campus Lunch Privilege Permission form must be filled out, signed by student and their parent/guardian.

Forms will be turned into Student Services to receive the off campus sticker.

Please note that along with this agreement any violation of Oregon City High School’s Code of Conduct could result in my off-campus privileges and/or parking lot privileges being revoked at any time with no refund.
